Activity Report Form
You must fill out an Activity Report Form if any of the following happen:
Equipment or vehicles are lost or damaged or any near misses.
Someone uses verbal abuse (e.g. shouting, swearing, or threatening language).
Any welfare incidents.
Any other type of incident occurs that does NOT involve personal injury or damage to a building.
Health & Safety Accident & Incident Report Form
You must fill out a Health & Safety Accident & Incident Report Form if:

Someone gets hurt or any near misses (including sporting injuries).

There is any damage to a building or any near misses.
Use this form to report things like sports injuries, slips, trips, falls, or anything else that causes harm to a person or building.
Expect Respect
Together with the University, we are standing against all forms of bullying and harassment; we Expect Respect. The University’s Report and Support tool provides a secure environment for those affected by harassment & bullying to speak out about unacceptable behaviour and to get support, or to find out how you can support someone you know who may have experienced harassment or bullying.
Report & Support
To ensure that every student has access to the resources they need, the University offers the “Report and Support” platform. This initiative is designed to empower students, staff, and visitors to report any concerns, from academic issues to incidents of harassment, bullying, or mental health struggles.
